Text Templates for No-Show Recovery and Patient Reactivation

A no-show text and a reactivation text are two different messages for two different problems. The no-show text goes out the same day to someone who had a time and missed it; its job is to rebook within the week. The reactivation text goes to someone who has not been in for months; its job is to restart a conversation, not to book on the spot. Below are copy-ready templates for both, a pre-appointment set that prevents the no-show in the first place, and rules for who gets which message and when.

The prevention set matters more than the recovery set. On about 160,000 appointments booked through KloudMD in the last 12 months, with reminders on, the no-show rate was 2.1% to 2.9% in every lead-time bucket, while cancellations ran from 3.9% for same-day bookings to 12.9% for bookings made two or more weeks out (KloudMD platform data, Sep 2025 to Sep 2026). If your no-show rate is well above 3%, fix the reminder cadence before writing a better no-show text.

Rules that apply to every template

  • Send from the number the patient already calls, so a reply lands in your inbox and not on a staff member’s personal phone.
  • No diagnosis, procedure detail, or test result in the body. “Your visit” or “your follow-up,” not “your biopsy results.”
  • One action per message. A link to pick a time, or a reply keyword, not both plus a phone number.
  • Every message names the practice. Patients get texts from many numbers.
  • Give a way out. “Reply STOP to opt out” on the first message of any outreach series.

Across the medical practices in our dataset, front desks send 2.4 outbound texts for every inbound reply (KloudMD platform data, Sep 2025 to Sep 2026). Most of that gap is reminders, which do not need a reply. The templates below are written to get one.

Set 1: preventing the no-show

Confirmation, within a minute of booking

One week before (only for appointments booked 2+ weeks out)

This is the message most practices do not send and the one the data argues for. Appointments booked 2+ weeks ahead cancel 3.3x more often than same-day ones, and nearly half of online bookings fall in that bucket.

48 hours before

Reminder from [Practice Name]: [Day] at [time] with [Provider]. Please bring your ID and insurance card. Reply C to confirm or R to reschedule.

Morning of

Today at [time], [First name]. [Practice Name], [short address or parking line]. Running late? Reply here.

Unconfirmed 24 hours out (no C received)

[First name], we haven't heard back about tomorrow at [time] with [Provider]. If you can't make it, reply R and we'll release the time for another patient. Reply C if you're coming. [Practice Name]

Naming the consequence (“release the time for another patient”) is what turns a silent no-show into a cancellation you can backfill.

Set 2: no-show recovery

Send the first message 30 to 60 minutes after the appointment start, the same day. Tone is neutral. Do not mention a fee in the first message even if you charge one.

No-show, same day

Hi [First name], we missed you at [time] today at [Practice Name]. Things come up. Here are the next openings with [Provider]: [link]. Or reply with a day that works and we'll book it.

No-show, no reply after 2 days

[First name], [Name] from [Practice Name]. Still want to get your visit rescheduled? Reply YES and I'll send two times, or tap [link] to pick one.

No-show, no reply after 7 days (last message in the series)

[First name], one last note from [Practice Name]. We'll keep your spot request on file. When you're ready, this link always shows real openings: [link]. Take care.

No-show with a fee policy (send only after the first message, and only if your policy is in writing and was agreed at booking)

[First name], per the policy you agreed to at booking, a [amount] missed-visit fee applies for [date]. We waive it when you rebook within 7 days: [link]. Questions? Reply here.

Second no-show

A second no-show should route to a person, not a template. Send one message, then have staff call.

[First name], this is [Name] at [Practice Name]. We've missed you twice and want to make sure the times we're offering actually work for you. What days and times are realistic? Reply here or call [number].

Set 3: reactivation

Reactivation is for patients with no visit in a defined window, typically 6 to 18 months depending on the specialty. Segment first. A patient who cancelled and never rebooked, a patient who finished a treatment plan, and a patient who has simply been well for a year each need a different opener.

Cancelled and never rebooked

Hi [First name], [Practice Name] here. You cancelled a visit with [Provider] back in [Month] and we never got you back on the calendar. Want to fix that? Tap [link] for current openings or reply and we'll help. Reply STOP to opt out.

Overdue for a recurring visit

[First name], it's been about [N] months since your last visit at [Practice Name], which means you're due. Here are [Provider]'s next openings: [link]. Reply with questions. Reply STOP to opt out.

Finished a plan, no follow-up scheduled

Hi [First name], [Name] at [Practice Name]. You wrapped up with [Provider] in [Month]. How are things going? If you'd like a check-in visit, here's the link: [link]. Reply STOP to opt out.

Long-dormant (12+ months), one-question opener

For patients out more than a year, do not send a booking link first. Ask a question a human can answer.

Hi [First name], this is [Name] from [Practice Name]. We haven't seen you in a while and wanted to check in. Are you still in the area and would you like to stay on our patient list? A quick YES or NO helps us. Reply STOP to opt out.

Anyone who replies YES gets the overdue template two days later. Anyone who replies NO is marked inactive. Silence after two messages means the number is likely dead; stop texting and try email once.

Reactivation follow-up (5 to 7 days after any reactivation opener, no reply)

[First name], following up from [Practice Name]. If now isn't the right time, no problem. If it is, here are two upcoming openings with [Provider]: [Day time] or [Day time]. Reply 1 or 2 to take one.

Timing the sends

Patients text practices mostly during the day. In a sample of more than 100,000 inbound texts, 83% arrived between 8am and 5:59pm, with the peak at 11am; by 5pm volume falls to less than half of the peak, and 6pm runs at about a third (KloudMD platform data, Jun to Sep 2026). Send reactivation openers mid-morning on a weekday, when a reply is most likely to be seen and answered by your team. Send no-show recovery the same day regardless of hour, since the trigger is the missed slot. Never send reactivation on a Sunday night.
